Birth Certificate
Immunisation History Statement
Available from the MyGov website. Dated within the last two months.
Proof of address; 2 documents required
- 1 document from the following;
Driver’s license, current rental agreement, current rates notice, current health care card,
Statutory Declaration – if living with someone in our catchment area.
We will also need one document showing their address.
If providing a Statutory Declaration, we will also need proof of ID of the person enrolling.
e.g. driver’s license or passport.
- 1 document from the following;
Current gas, water or electric bill.
If not a permanent resident of Australia
Copy of passports and visa sub class document.
If there are any Family Court Orders or Violence Restraining Orders;
We require a copy of the current orders in place.
We are a local intake school. Out of area applications will be considered.
However, there will be no guarantee they will be accepted.
